The Hull Daily Mail's Health and Care Awards closes for nominations on 1 September.
Have you experienced outstanding healthcare? Would you like to nominate a health professional or carer for an award? Categories include:

  • Outstanding Health Professional of the Year - recognises a person who shows immense dedication to duty, providing the best possible care for patients in a community or healthcare environment.
  • Carer of the Year - to reward someone looking after a relative who always put their needs first.
  • Young Carer of the Year - for a young person under 18 who dedicates their time to caring for a parent or relative.
  • Care Home of the Year - where staff put residents at the heart of everything they do, treating people with the utmost dignity and respect and tailoring excellent services to an individual’s needs.
  • Volunteer of the Year - for the someone who gives up their own time to visit the lonely or frail or to improve the health of people in their community.
  • The Mental Health and Wellbeing Award - will reward a service or an individual offering support, advice or help in the community to encourage wellbeing. It could be a dementia champion playing a key role in helping people with memory loss or a service tackling depression or anxiety.
  • Health Improvement Award - For the award tell us how a project in your community is changing people’s lives for the better.
  • Innovation in Health and Social Care Award - aims to highlight innovation putting East Yorkshire on the map.
  • Health Partnership of the Year Award -  are you a charity working with the NHS or are you part of a public sector organisation working in partnership with the health service?
  • Our Unsung Health Hero Award - the person working outside the health service or public sector who shows real dedication and compassion in care, bringing about real change or ensuring others always come first.
