A new 8-page newsletter – My City, My Health, My Care has been produced to share some of the developments taking place in your GP services.

Demand on general practice in the UK has intensified in recent years. An ageing population and increasing numbers of people with complex conditions mean that GP services in Hull must evolve to meet these new demands.

The My City, My Health, My Care newsletter contains information on the CCG’s Primary Care Blueprint; a plan which lays out the possible developments in GP Services.

The newsletter also contains a profile of the practice teams who support your GP, information on premises and explains how you can share your thoughts and get involved in shaping the future of GP services in Hull.

The newsletter is available to pick up from dozens of local venues including you GP surgery, pharmacy or library. Alternatively, the newsletter can be found online at our website. You can read it here.

NHS Hull CCG is committed to transforming primary (GP) care for patients in Hull in a way that supports local practices to address the challenges they face in continuing to deliver high quality, sustainable services.
